COMPOSITION:
Verapamil hydrochloride 5 mg/2 ml injection solution,
80 mg in one filmtablet.

ACTION:
The drug belongs to the group of calcium antagonists. The mechanism of its action is associated with blocking of calcium ions influx through the slow calcium channels in cellular membranes. It suppresses the activity of the sinus node and slows down AV conduction predominantly. It has a local anesthetic activity similar to that of procain. It induces peripheral and coronary vasodilatory effect as a result of the inhibitory action on the smooth muscles of the vessels. The drug has a negative chrono-, dromo- and inotropic effect. It reduces the oxygen demand of the myocardium.

PHARMACOKINETICS:
Verapamil is well absorbed in the digestive tract and is subjected to biotransformation in the liver into less active metabolites. About 90 % of the drug binds to blood plasma proteins. With oral administration the effect is attained after 1/2-2 h and lasts for 6 h. its plasma half-life is 3-4 hours.

INDICATIONS:
Supraventicular arrythmia (paroxysmal tachycardia, arterial extrasystoles and paroxysmal arrhythmia); angina pectoris, light and moderately grave hypertension, in combination with other hypotensive agents and in hypertrophic cardiomyopathies.

CONTRAINDICATIONS:
Cardiogenic shock, acute myocardial infarction complicated with bradycardia, hypotension, left-venticular insufficiency, AV-syndrome); uncompensated cardiac insufficiency; marked bradycardia and arterial hypotension; pronounced cardiac insufficiency, as well as during the first three months of pregnancy.

SIDE EFFECTS:
Dizziness, nausea, and in high doses - hypotension, disturbances in consciousness are likely to be observed, as well as sinus bardycardia that can develop into asystole and AV-block.

PRECAUTIONS:
Infusion with catecholamines, 10 % solution of calcium gluconate, atropine are administered in toxic manifestations, and when necessary - cardiostimulation.

DRUG INTERACTIONS:
The combination of verapamil with amiodaron or with betadrenoblockers is not recommended because of its depressive effect on the cardiac muscle. Verapamil in combination with cardiac glycosides is recommended, because their toxicity is reduced, and for avoiding the spasm of the vessels caused by them.

DOSAGE AND ADMINISTRATION:
Adults - oral treatment of stable angina pectoris and arrhythmia starts with a dose of 40 mg, three times daily, increasing gradually until the therapeutic effect is attained; in unstable angina pectoris and hypertension the doses are higher by about 20-30 %. Lower doses are administered in case of disordered hepatic function.
Children - with doses from 20 to 80 mg, 2-3 times daily, depending on age and body weight.
The injections are administered intravenously in doses from 5 to 10 mg; the dose can be repeated after 30 min if no effect is obtained.

MEDICINAL FORM AND PACKAGE:
Packages with 10 ampoules of 5 mg/2 ml.
Packages with 30 filmtablets of 80 mg.

STORAGE:
At moderate temperature (15-30° C), in a place protected from light.

EXPIRY:
Ampoules - 2 (two) years.
Filmtablets - 3 (three) years.
