- COMPOSITION:
Epinephrine tartarate
- 0,25 mg/1 ml
0,5 mg/l ml in one ampoule
ACTION:
Epinephrine is a hormone secreted by the adrenal medulla. It
excites the alpha- and beta-adrenoreceptors. Increases the tone
of the smooth muscles of blood vessels and induces
vasocostriction in the abnominal cavity, skin, muscous membrane;
it has no effect on the tonus of the pulmonary and cardiac blood
vessels. It elevates the level of blood sugar and intensifies
tissue metabolism. It elevates the blood pressure abruptly but
for a short time, excites the respiratory center, reduces the
tonus of the smooth muscles of the bronchi and intestines. It
reduces their peristalsis, as well as that of the stomach. Due
to its effect, the oxygen need of the myocardium rises sharply,
the electrolyte metabolism is disturbed and conditions are
created for the appearance of arrhythmias. It slightly excites
the central nervous system. It is rapidly destroyed under the
effect of monoaminooxidase and other enzymes in the body.
INDICATIONS:
An analeptic in shock and collapse; in circulatory insufficiency due
to intoxications, infectious diseases, suffocation.
In sudden cardiac arrest.
In bronchial asthma (rarely nowadays).
In allergic reactions, serum sickness, hypoglycemic coma.
As local haemostatic and vasoconstricting agent.
CONTRAINDICATIONS:
Epinephrine should not be used in anaesthesia with ftorothan,
cyclopropane, chloroform, chlorethyl due to the risk of venticular
fibrillation. Do not administer in collapse caused by massive blood
loss; in preliminary administration of phenothiazine neuroleptics,
hypertonic disease, thyrotoxicosis. |
SIDE EFFECTS:
With the usual doses, some patients react with short-term paleness,
tremor, sense of fear, palpitation, dizziness, perspiration,
difficult miction. The patients with hyperthyroidism are especially
sensitive.
PRECAUTIONS:
Nitrates (amylnitrate or nitroglycerine) administered as fast as
possible are the reliable therapeutic agents in epinephrine
intoxication.
DRUG INTERACTIONS:
Reduction of blood pressure is observed with alpha-blockers; with
beta-receptor blockers - bradycardia, sometimes paradoxical
elevation of blood pressure; disturbed cardiac rhythm is observed
with cyclopropane, halothane, isoprenaline; reduced insulin effect;
phenothiazine and saluretics diminish the action of epinephrine;
thyroid hormones enhance its action.
DOSAGE AND ADMINISTRATION:
It is administered in very low doses: subcutaneously - initially
0.1-0.3 mg (0.1-0.3 of 1% solution). The dose can gradually be
increased to 0.5 mg; intravenously - very slowly, not more than 0.2
mg as an additive in procaine administration; 1 drop of 1%
epinephrine is added to 1 ml procaine; topically - in capillary
haemorrhage, rhinitis, conjunctivitis (0.2 %).
MEDICINAL FORM AND PACKAGE:
Packages with 10, 50 and 100 ampoules of 0.25 mg/1 ml and 0.5 mg/1
ml.
STORAGE:
At moderate temperature (15-30° C) in a place protected from light.
EXPIRY:
2 (two) years.
