COMPOSITION:
Atropine sulfate 0.5 mg
Codeine phosphate 20 mg
Promethazine hydrochloride 15 mg
Noramidopyrine methansulfonate sodium
in one ampoule of 2 ml 500 mg

ACTION:
The combination of the spasmolytic action of atropine with the pain-relieving properties of codeine and analgin, complemented by the central sedative and hypnotic effetc of promethazine, brings the drug near to morphine in the intensity of pain-relieving action.
The risk of addiction to the drug, however, is far lower than to morphine and to the synthetic analgesics of the morphine group.
Analgin is quickly absorbed and, in fact, completely with peroral administration and 50% of the maximum concentration is attained after 30 min. half-elimination time is about 7 h, 90% of the elimination is realized through renal excretion.
Codeine is metabolized in the liver by glucuronization and thus it has an enhanced toxicity in childhood. It is mainly excreted with the bile, with a mean coefficient of elimination of 0.3-0.7.
Atropine absorption depends on the pH-values of the medium, and when the pH-values of the gastric juice are low, absorption is slower in the stomach and quicker in the intestines. Its binding to plasma proteins ranges within 30-50%, promethazine binding being only 7.5%.

INDICATIONS:
Acute pain syndrome with concominant spasms of the smooth muscles: ulcer disease, biliary or renal colic. Neuralgia, post-operative pain, in oncological practice.

CONTRAINDICATIONS:
Low blood pressure.
SIDE EFFECTS:
Fast blood pressure drop is likely to occur after intravenous injection.
 
PRECAUTIONS:
To be injected when the patient is in lying position. Due to its hypnotic effect, the drug must be administered under hospital conditions or at home. Prescribing the drug to outpatients should be avoided! After its administration (irrespective of the form), the patient should be forbidden to practice activities requiring fast reflexes and sharp attention for several hours (driving and machine operation).

DOSAGE AND ADMINISTRATION:
One ampoule, 3-4 times daily during strong pain - subcutaneously or intramusculary. The intravenous injection should be slow when the patient is in a lying position.
Orally - 1-2 filmtablets, one or two times daily.

MEDICINAL FORM AND PACKAGE:
Ampoules of 2 ml in packages of 10, 50 and 100.
Filmtablets in packages of 20.

STORAGE:
At moderate temperature (15-30 C), in a place protected from humidity.

EXPIRY:
2 (two) years.
