Drug form and composition
Film-coated tablets. One film-coated tablet contains: 150 mg Calcium hydrogenphosphate
dihydrate; 70 mg Calcium lactate; 50 mg Calcium glycerophosphate; 20 mg Ascorbic
acid.
Indications
Diseases accompanied by a calcium-phosphate balance disorders - hypocalcemia
of different causes, osteomalacia, allergy predisposition, convalescence following
severe disease; fractures, etc. In elderly patients with osteoporosis, for normalization
of the calcium-phosphate exchange. Conditions with enhanced calcium demands - pregnancy,
lactation, accelerated body growth, etc. In patients with diabetes, thyrotoxicosis,
and following corticosteroid therapy for prevention the changes in bone structure.
Dosage and administration
In adult patients - 1-2 tablets, 3-4 times daily. In children over 6 years -
1 tablet 2-4 times daily. The dosage and duration of the treatment is determined
by the doctor, depending on the severity of the disease. If necessary the preparation
may be used in long-term treatment courses or short courses at intervals of several
mounts.
Contraindications
The preparation should not be administered in patients with accelerated blood
clotting; thrombophlebitis; predisposition to thrombosis; hypercalcemia; arteriosclerosis;
chronic renal failure; severe liver disorders; prosthetic cardiac valves; glucoso-6-phosphate
dehydrogenase deficiency; nephrolithiasis and hyperoxaluria; thalassemia.
Special warnings and precautions
Calcium phospho C should be used with care in persons with disturbed coagulation,
as in such cases long-term administration requires monitoring of the coagulation
status. During the treatment with the preparation is advisable to determine serum
concentration of the phosphates, as well as to investigate the functional status
of the kidneys. In patients with diabetes the medicine should be used carefully,
as a risk of deterioration of the functional activity of the pancreatic beta-cells
exists. In long-term application glucose level should be monitored.
Drug interactions
Calcium improves the action of the digitalis and elevates sensitivity to this
medicine; reduces effectiveness of some local anesthetics. Concomitant administration
of calcium preparations and tetracycline antibiotics reduces the absorption level
of the later in the gastrointestinal tract, because of the formation of nonabsorbable
complexes with calcium. Calcium preparations antagonized neural-muscular block,
caused by amikacin and other amynoglycoside antibiotics. In concomitant use with
barbiturates the excretion level of the later is reduced, their plasma level is
elevated, and its pharmacology and toxic activity are intensified.
Adverse reactions
Most frequent adverse reactions are gastrointestinal manifestations - nausea,
vomiting, gastrointestinal discomfort in patients with predisposition.
Pharmacological mechanisms
Calcium regulates membrane permeability, neuromuscular excitation, blood clotting,
activating of several enzyme systems, and participates in the muscular contraction.
Phosphorus plays a basic role in formation of the high energy compounds, including
ATP and creatinphosphate; participates in formation of the bones and teeth. Calcium
and phosphate metabolism in the organism is regulated by the parathyroid hormone
and calcitonin. Calcium glycerophosphate, containing calcium and inorganic phosphate,
stimulates mineralization of the teeth tissue and tubular dentine formation. Data
exist that oral calcium possesses preventive effect concerning intestinal carcinogenesis.
Ascorbic acid participates in the synthesis and exchange of the steroid hormones,
and adrenaline; stimulates formation of the collagen structures in the bones, muscles
and teeth; normalizes tissue exchange and respiration; activates prothrombin. It
possesses anti-toxic and antioxidant activity. Reduces the enhanced permeability
of the capillary vessels, and limits the exudative, inflammatory, and allergy reactions.
As an antioxidant ascorbic acid may regulate directly free-radical reactive oxygen
molecules. It stimulates the regeneration of the tissues and the growth.
